Focus Assist/Do Not Disturb Keeps Randomly Turning Itself On and Off

I have no need for this feature and have never set it up, yet today while playing a game, it randomly turned itself on and off. I went into my settings and disabled everything, but it keeps happening. How do I make it stop? How can I get rid of this feature entirely, as I don’t want or intend to use it? I've followed every guide I found, and none of them have helped. The sound it makes when it activates and deactivates is driving me insane, and there’s absolutely no reason for it to be doing this.

I do NOT have any DND settings checked. I do NOT have any Focus Assist settings checked. They are all unchecked, yet this keeps happening.
